USPS Form 1583

USPS Form 1583 authorizes Digital Post Office to receive mail on your behalf. This form is required for all private mailbox customers.

Important

USPS Form 1583 must be completed, signed, and notarized before mail can be released. Incomplete or incorrect forms will delay service.

How to Complete USPS Form 1583

Follow the steps below carefully. All information must match your identification.

Enter your full legal name and current physical address. PO Boxes are not accepted as a physical address.

If this mailbox is for a business, enter the legal business name and entity type. Individuals may leave this section blank.

Two forms of valid identification are required. At least one must be a government-issued photo ID.

  • Driver’s license or state ID
  • Passport
  • Vehicle registration
  • Voter registration card

List any additional individuals authorized to receive mail for this mailbox. Each authorized individual must present identification.

The form must be signed in the presence of a notary. Do not sign the form until you are with the notary.
